Care Management

Care Management is a member of the healthcare team for patients in acute rehab. A care coordinator (RN) and a social worker (SW) will perform an assessment to identify the patient’s needs and coordinate the agreed upon discharge plan to ensure a smooth transition home or to another level of care. They serve as patient/family advocates and assist with resources as needed.

Registered dietitians evaluate a patient’s nutritional needs and preferences, develop appropriate plans and provide diet instruction. Please let your dietitian or nurse know if you have any food allergies, religious restrictions, special requests or other dietary needs.

Chaplain Services

Parkland’s chaplains can offer you and your loved ones spiritual and emotional help during your stay. Chaplains care for people of all faiths and backgrounds and are here 24 hours a day, every day. If you want to speak to a chaplain, please let your nurse know. The chaplain is part of your healthcare team and can be a listening ear, can help you use your faith as you cope with being in the hospital and can also guide you as you have to make hard choices. The chaplain can also help you understand and fill out advance directives. If you have your own faith leader we can help you get in touch with them or we can try to find a person of your faith group for you to talk with.

Orthotics and Prosthetics

Orthotists and prosthetists design and apply devices that help the body work better. These devices can help our patients to live healthy, productive, and independent lives.
