Birth Certificates

The Birth Registry Office at Parkland Health obtains information from newborn parents to file a birth certificate with the Texas Department of State Health Services Vital Statistics Office. The Birth Registry Office can provide a Verification of Birth Facts for newborns, and Acknowledgement of Paternity forms. Any questions about the completion of birth certificates for babies born at Parkland should be directed to the Birth Certificate Office at Parkland at 469-419-4902 or 469-419-5625.

• Requesting the original birth certificate

  Parents can get the original birth certificate by contacting the
  Bureau of Vital Statistics.
