Apply for Parkland Financial Assistance

New to PFA


If you have never applied for Parkland Financial Assistance (PFA) or it has been more than 6 months since your PFA expired and you would like to see if you qualify, follow these steps to apply.

Online:

Apply online here or through your Parkland MyChart.

 

By mail or in person:

  1. Please complete the application. (English/Spanish)
  2. Please complete the employment verification form. (English/Spanish)
  3. Please complete the asset form if you have Medicare (English/Spanish) and/or the self-employment income form (English/Spanish) if you are self-employed.
  4. Please complete the verification of support form if someone else provides you financial or residency assistance. (English/Spanish).
  5. Gather the documents needed. Parkland requires:
    • Proof that you live in Dallas County (residency)
    • Valid photo identification (ID)
    • Proof of Income
    • Number of people in your household
  6. Review the required document list. (English/Spanish)
  7. Return the completed application and copies of all documents. You can return the forms the following ways:
