Apply for Parkland Financial Assistance
New to PFA
If you have never applied for Parkland Financial Assistance (PFA) or it has been more than 6 months since your PFA expired and you would like to see if you qualify, follow these steps to apply.
Online:
Apply online here or through your Parkland MyChart.
By mail or in person:
- Please complete the application. (English/Spanish)
- Please complete the employment verification form. (English/Spanish)
- Please complete the asset form if you have Medicare (English/Spanish) and/or the self-employment income form (English/Spanish) if you are self-employed.
- Please complete the verification of support form if someone else provides you financial or residency assistance. (English/Spanish).
- Gather the documents needed. Parkland requires:
- Proof that you live in Dallas County (residency)
- Valid photo identification (ID)
- Proof of Income
- Number of people in your household
- Review the required document list. (English/Spanish)
- Return the completed application and copies of all documents. You can return the forms the following ways:
