Parkland Employee/Staff Daily Health Screening
This screening tool is designed to identify your risk of passing COVID-19 from person to person at work. Please do it every day and answer truthfully to protect you and those around you.
Please complete the screen within 4 hours of your scheduled shift, and early enough to meet your area’s policies on timely notification of illness to your supervisor. If you have traveled to a COVID-19 hotspot, please identify as having traveled in the survey.
