Health services for seniors

Parkland’s Geriatric team provides many services for patients age 65 and older. Our Geriatric staff understands the special medical needs of seniors and is specially trained to manage their health concerns and help them stay healthy.

Geriatric Primary Care Clinics

Parkland has geriatric primary care clinic locations across Dallas County so that seniors age 65 and older can get healthcare near their homes. We also help with transportation problems and connect them to support services in their community.

Geriatric care includes routine health checkups, sick care and management of chronic diseases. Our doctors, nurse practitioners and nurses specialize in care for older adults. We also assign team members to help older adult patients and their caregivers with extra care and connection to community services.

Senior HouseCalls Program

Senior HouseCalls provides primary medical care to homebound patients age 65 and older. Care is provided in the patient’s home where staff can best work with family members and community resources, like home healthcare, as needed.

This program provides:

• Medical care
• Social work assessment
• Referral to community agencies
• Nutrition support

The Senior HouseCalls care team includes doctors, a nurse practitioner, social worker, nurse and chaplain. Help from a clinical pharmacist, dietitian and financial counselor can also be arranged.

Parkland Senior Outreach Services

Parkland Senior Outreach Services cares for adults age 65 and older who live in the south and southeast areas of Dallas County. The program is funded by a grant from the Texas Department of State Health Services. Four key services are:

• In-home review of the patients’ needs, referrals to community agencies and health education.
• Free health education for older adult groups, including:
• Community education on topics about aging (for both professionals and seniors)
• Educational classes about diseases, such as arthritis
• Health outreach and screenings
• Limited van transportation for older adult patients living in ZIP codes 75215, 75216, 75217 and 75241.
